Short answer Yes
Moonshot EHR has a forms builder for intake questionnaires, consents and program agreements. Patients complete and sign them in the portal, conditional questions show only when relevant, individual answers can stop a patient and flag the clinic, and consents can be signed in the office on the clinic's device. Signed copies live in the chart.
How it works in Moonshot
- Build templates with staged sections and show-if logic, then send them to a patient, attach them to an appointment type, or publish a public intake link.
- Per-question flags: a chosen answer can stop the patient from continuing and raise a flag the clinic sees in review.
- Program agreements pull the product's price and terms into the document with tokens, freeze them when sent, and record the portal signature as consent.
- Refill questionnaires and patient-reported draw dates feed the refill workflow instead of a phone call.
Limits, stated plainly
- Forms are Moonshot's own; there is no import of a Jotform or Typeform definition, though a public intake link can replace them.
- Conditional logic is show-if per question or section. There is no scoring engine or branching across separate forms.

Questions people ask next
Can a new patient fill out intake before their first visit without a portal account?
Yes. A public intake link or the booking flow collects intake first; the portal account follows.
Can we require a consent before a specific appointment type?
Yes. Forms can be attached to appointment types, and staff see what is outstanding on the visit.
Are signatures legally binding?
Signatures are recorded with the signer, time and document version as consent records. Whether a given form satisfies your state's rules is a question for your counsel, not the software.
