Best EHR for Cash-Pay Clinics in 2026

Nine EHRs that cash-pay clinics actually shortlist, what each one is built for, and the questions that separate them. Written by the founder of one of them, with the other vendors’ facts taken from their own pages.

By Tom Kashul, Founder, Moonshot EHR; clinic owner-operator

Moonshot wrote this guide. I founded Moonshot EHR and run a cash-pay clinic on it, so read the Moonshot entry with that in mind. Every other vendor’s facts come from its own pricing, feature and help pages, checked on October 19, 2026 and listed under Sources. Where a vendor does not publish a price or a feature, the table says so instead of guessing. Each entry says who the product fits, including when that is not a Moonshot customer.

A cash-pay clinic is one where the patient pays the clinic: memberships, packages and visit fees, with no insurance claims. That changes what matters in an EHR. Claims scrubbing and payer rules stop mattering; membership billing, your payment processor, texting and, for prescribing clinics, EPCS start to. Moonshot EHR does not bill insurance at all, and several products below do. If you bill payers, weigh them on that.

The nine, and who each one fits

Moonshot EHR

Fits a cash-pay clinic that prescribes, especially TRT, hormone, GLP-1 and longevity clinics, and wants memberships, e-prescribing with EPCS for Schedules II through V, two-way texting, fax and medication inventory in one system. Medical is $1,000 per location per month with one prescriber, $200 per additional prescriber and unlimited staff, with no platform transaction fee; Performance, for DEXA and testing businesses, is $650. It does not bill insurance at all, and for a single prescriber who does not dispense or run memberships it costs more than most tools on this list.

Cerbo

Fits functional and integrative medicine practices that want a long-established vendor with per-provider pricing: $281 per full-time prescribing provider per month, $63 per staff user and a $1,195 setup fee. It e-prescribes including controlled substances, runs subscriptions and recurring payments, and includes outbound fax, with inbound fax as an add-on. Two-way texting comes from third-party services.

Practice Better

Fits non-prescribing wellness practices: nutrition, health coaching and most naturopathic work, at $35 to $155 a month. E-prescribing is a paid add-on per prescriber, and two-way SMS is not listed. If nobody in your clinic prescribes, it is far cheaper than a medical EHR.

Healthie

Fits nutrition, coaching, behavioral health and virtual-first practices, and teams that want an API. Plans run $19.99 to $129.99 a month billed monthly, with a 14-day trial of Plus. E-prescribing is a DoseSpot add-on at $40 per provider per month, and EPCS is on the Enterprise plan only. Packages bill on a recurring schedule, insurance claims run through Claim.MD, and its help center says clients cannot reply to its reminder texts.

Jane App

Fits physiotherapy, chiropractic, massage and other appointment-based allied health clinics, at $59 to $99 a month plus per-practitioner licenses. Memberships are on the Thrive plan. Jane announced US e-prescribing with EPCS through DoseSpot in October 2026, in early access, so check its status before you plan on it.

Hint Health

Fits direct primary care and concierge practices whose business is the membership, including employer-sponsored and hybrid plans. Membership billing is the core product, Hint Clinical is its EHR, and every plan includes unlimited users; Launch starts at $290 a month with one clinician. Payments run on Hint Payments at published rates. Hint’s pages list no insurance claims product and no controlled-substance inventory.

Tebra

Fits independent practices whose revenue runs through insurance: claims, eligibility and a clearinghouse are the product, with an AI voice assistant and an AI note tool on top. Pricing is quoted per clinical provider. Its help center says recurring monthly charges cannot be set up in Tebra Payments, which matters for any clinic that runs on memberships.

Elation Health

Fits independent primary care, including direct primary care practices that may add insurance later: Elation Billing handles claims, and Membership Management bills recurring plans on Stripe with a 2.99% fee per payment. Pricing is quoted. Built-in texts are reminders; patient texting conversations run through the Spruce integration.

CharmHealth

Fits a practice that wants a low entry price and pays per feature: a free plan, a per-encounter plan, and a $200 per provider Provider plan with unlimited users, with add-ons for eRx, EPCS, two-way texting, fax and a website booking bot. It also bills insurance. Card payments and recurring charges run on the Bluefin gateway only.

Side by side

Rows are the questions cash-pay operators ask most. Cells come from each vendor’s own pages as of October 19, 2026; "Not listed on vendor site" means we could not find it on the vendor’s pages, not that it cannot be done with a third-party tool. The one-on-one pages for Cerbo, Practice Better, Jane App, Tebra, Elation Health, CharmHealth and athenahealth carry more rows.

How to choose

Do I need to bill insurance at all?

Decide this first, because it removes half the list. Moonshot EHR does not submit claims, and Hint’s pages list no claims product. Tebra, Elation, CharmHealth and Healthie do, and athenahealth is built around it. If a meaningful share of your revenue must come from payers, pick an insurance-capable system; a cash-pay EHR plus a separate billing service is a workaround, not a plan.

How does membership billing actually work in it?

Ask to see a membership renew on a card on file, a failed payment retried, and what the plan includes per period. Moonshot, Cerbo, Healthie, Hint, Elation and CharmHealth all bill recurring charges in some form; Jane sells memberships on its Thrive plan; Tebra’s help center says recurring monthly charges cannot be set up in Tebra Payments. Also ask whether patients can join from a public page, and what happens to memberships when you migrate (memberships moving to Moonshot from Cerbo or Athena are rebuilt by staff, and payment history does not carry over).

Are texting and fax built in, or another subscription?

Built in means the EHR owns the number and the reply lands inside the EHR, not in another app. Moonshot and CharmHealth list two-way texting themselves (CharmHealth as a paid add-on); Tebra lists two-way text communication; Cerbo and Elation point to third-party apps (Curogram, Spruce) for patient texting; Healthie’s reminder texts cannot be replied to; Jane and Practice Better send reminders. Fax is built in or an add-on on most of the list; check inbound as well as outbound.

Can my prescribers send EPCS from day one, and what does it cost?

EPCS is the line between a wellness platform and a prescribing EHR. On Moonshot it is part of the Medical plan through ScriptSure. CharmHealth charges $250 per provider per year, Healthie limits it to Enterprise, Practice Better sells e-prescribing as an add-on, Elation says EPCS may cost extra, Tebra charges a $75 application fee, and Jane is in early access. On any system, each prescriber completes identity proofing before the first controlled script, and that takes real days, so start before opening day.

Can I bring my own payment processor?

Hormone and GLP-1 clinics are often underwritten as higher risk, so being locked to one processor can stall an opening. Moonshot runs on Stripe, NMI or TRX on your own merchant account (NMI or TRX adds a $200 per location module). Cerbo lists Propelr, Bluefin or Stripe. Most others route through their own payments product or one partner: Tebra Payments, Jane Payments, Hint Payments, CharmHealth on Bluefin, Healthie and Elation on a Stripe account created through them. Underwriting is always between you and the processor; no EHR can promise approval.

Is it priced per location or per provider?

Do the math with your real headcount. One location with three prescribers and four staff: Moonshot Medical is $1,000 plus 2 × $200, or $1,400 a month. Cerbo is 3 × $281 plus 4 × $63, or $1,095 a month, plus its setup fee. CharmHealth’s Provider plan is 3 × $200, or $600 a month before add-ons. Hint Launch is 3 × $290, or $870. Tebra and Elation quote. Per-location pricing gets cheaper per head as you add staff; per-provider pricing is cheaper when you are small.

Where Moonshot fits, honestly: a clinic that prescribes, runs memberships, and wants texting, fax, EPCS and inventory in one system at a per-location price, plus $200 per additional prescriber. If you do not prescribe, a wellness platform costs less. If you bill insurance, Moonshot is the wrong tool. See Moonshot pricing or watch the demo.

Frequently Asked Questions

What is the best EHR for a cash-pay clinic?
It depends on what the clinic does. For a cash-pay clinic that prescribes, including controlled substances, and runs on memberships, the shortlist is an EHR with EPCS, membership billing and texting built in, which is what Moonshot EHR is built for. For non-prescribing wellness work, Practice Better or Healthie cost far less. For direct primary care built on memberships, Hint and Elation are common choices. If the clinic bills insurance, Tebra, Elation or CharmHealth fit better than any cash-pay-only system. Moonshot wrote this guide.
Which EHRs for cash-pay clinics publish their prices?
As of October 19, 2026, per vendor pages: Moonshot EHR, Cerbo, Practice Better, Healthie, Jane App, Hint Health and CharmHealth publish subscription prices. Tebra and Elation Health quote per practice.
Does Moonshot EHR bill insurance?
No. Moonshot EHR does not submit insurance claims, run eligibility checks or connect to a clearinghouse. It is built for cash-pay and membership clinics; patients get itemized receipts they can submit to an HSA, FSA or out-of-network benefit themselves.
Which EHRs include two-way patient texting?
Per vendor pages as of October 19, 2026: Moonshot EHR includes two-way texting from a clinic number; CharmHealth sells it as an add-on; Tebra lists two-way text communication. Cerbo and Elation Health point to third-party apps for patient texting, Healthie says clients cannot reply to its reminder texts, and Jane App and Practice Better send reminder texts.
Who wrote this guide?
Tom Kashul, founder of Moonshot EHR and owner-operator of a cash-pay clinic that runs on it. Moonshot is one of the products compared. Competitor facts come from each vendor’s own pricing, feature and help pages, checked on October 19, 2026, with sources listed at the end of the page.
